Use the online form below to report your weekly results,

or email results to scores@qballpoolleague.com

Form Instructions:

 

· enter the names of the HOME and AWAY  teams in the drop-down boxes and note whether your team played at home or away

· enter your NAME and PHONE NUMBER

 

· for each game fill in the name of the Away team player and Home team player, and indicate whether the Away player won, lost, won due to forfeit, or forfeited.  (for forfeit games, list absent player’s name as ABSENT or N/A)

 

· enter the overall team scores  AWAY/HOME from the drop-down menu

· enter information on any SUBSTITUTES that played

· enter any COMMENTS

· enter your EMAIL address and click SUBMIT

 

Enter the code number and submit …….. wait for the progress bar on the following page, then click “continue” at the top to return to QBall home page.

 

 

This form’s layout follows the team booklet format: Away player, W/L, Home Player. But its kinda clunky…..if anyone can come up with a more user-friendly form (in HTML) please contact me.
